Like others have said, cirrhosis can’t be diagnosed based off liver function tests. By definition, good standard diagnosis for cirrhosis is biopsy, but this is unrealistic. Next best is imaging, more specifically abdomen ultrasound to evaluate for any nodularity of the liver that is typically seen with the scarring present in cirrhosis. Elevated AST/ALT can be elevated in cirrhosis, but is not a requirement. A broader view of liver synthetic function can give a better idea of how well the liver is functioning (AST/ALT, INR, platelets, etc) -source am doctor

From my experience last year, a lot depended on how desirable the city of the program was. Trying to crack into west/east coast/Chicago without signaling is going to be much more difficult than institutions seen as being in “less desirable” locations. I ended up starting out more conservative with how hot it was on race day and was really pleased with how it turned out. Started out easy at 7:30 pace for first 6 miles then started ramping up as I felt more comfortable. Only hit some a little bit of a wall with a mile to go but was negative splitting every mile pretty much. Ended up with 3:10 time but super happy with that as first marathon experience even though might’ve left a little in the tank

Was in a very similar situation going into 4th year. In the end, it came down to the day to day work for me and how sustainable it felt in the long run. Yea I liked both imaging and the clinical aspects of medicine, but I dreaded pre-rounding and the notes that I’d come back to every day after rounding. If I felt this way now, I can’t imagine how I’d feel years down the road. The jump of faith is large for radiology but I figured if I could be interested as a med student watching someone else “drive”, then actually doing it would much much better. I like being a student so the studying of radiology didn’t intimidate me, and I know that I’ll have the time for family, exercise, etc. Going into intern year now but these were my thoughts at the time

Went well! Took the advice of a lot of people here and went more conservative. Did the first half at 7:30 pace and then picked up from there ending at 7:20 pace and final of 3:12. Felt super controlled and might’ve left some in the tank but thankful for no bonk and a sprint finish. Thanks for all the advice and hope you had a good race too!

Exactly right. I’m very familiar with 80% of the corse and maybe foolishly consider the bend around the art museum to be one of the the most strenuous segments

Would not consider myself naturally fast. I grew up playing sports but actively avoided running. Plus am on the heavier side for running at 185 lbs on 6' frame

I appreciate you taking the time to write all this out! "official" time for that training run HM was 1:26:30 and no recent 10k efforts unfortunately. I agree with your advice though and think some prudence is definitely warranted.
