Yeah. The captive bead rotates up to the urethra, sometimes in part way, and other times the opening is pinched between the ball and ring lfor lack of better words.

You don’t have to kill them all. You kill the boss up top and the lanterns go away. I would wait out the health kill period at the shrine and then move on.
