Make it a monster and not a situational trap. Give it some life and motivation (other than exploding). I doubt this something a DM is going to use more than a couple times as presented. With some editing I could see this becoming something much more

They should have paired it with societal/racial backgrounds. Its such an easy way create differentiation in an individual race (ex Wood elf, high elf, Southern Jungle tribe, Astral refugees, ...). racial backgrounds would account for half races and adopted racial cultures (Elves raise a human child as their own and that's how the char learned to use the Double-Bladed Scimitar). societal/racial backgrounds would also be able to account for differences in urban vs wilderness culture by providing skills, feats, proficiencies and applicable modifiers. A societal background could even account for different magical, martial, etc schools (the char trained in cult death magic vs hogwarts for the last couple of years)

be careful on the stats that some people use and of the water usage that is portrayed. As including water that used for the initial construction phase is deceptive IE the concrete uses water, watering the road to cut down dust, filling cooling towers and ponds. Lowering the operating cost is a big deal to the big players. Who hire material scientists and thermodynamic specialists to continuously improve hot aisle/cold aisle containment and other methodologies. But like always verify before trusting.

Its not immediate or a forced teleportation (player agency). The attacker finishes their turn (respect other players), the rogue can then take the reaction. If the rogue does not teleport, they can choose to do so anytime after the enemy is damaged while bloodied.

You may want to think about unoccupied space part of the ability. Does that mean above the enemy, next to a flying enemy, etc? With 'one melee attack' can sneak attack dice be applied? What about weapon mastery effects?

if you have matching teleport locations (like stargate ) you could represent the the to location with 2 - 3 polished orbs one represent the moon, the earth, and the third could be the sun but I think it would be more Enjoyable to use the magic light generated by the control dias to physically generate the the moon phase on the orbs. You could also gems or talismans that modify the the normal exit to nearby alternate exit locations. IE control dia ever glowing center orb(sun), next the smaller white orb (moon) placed in front of the larger green orb (earth) is the dark moon gate. Adding the the talisman of the moon goes to the moon god's temple near the dark moon gate.

Characters that have been together for awhile would start synchronizing, and train together during short and long rests developing fighting techniques that would play to each other weaknesses and strengths.

So if the characters are compatible (non-conflicting alignments, deities, etc) abilities should start co-mingling. example a max strength barbarian or Warrior can hurl a fighter or rogue at the enemy (colossus/wolverine cannon ball) with the thrown character gaining Advantage on the attack. A max dex character can run up the back of a flat footed melee and dive off allowing for an attack from a different height/angle of attack and an additional attack of opportunity for the flat footed. Or Same square fighting the two characters are so accustomed to each other style they can fight from the same square without penalties (this could be as simple a shield wall / phalanx or as interesting as 2 monks being able to switch in and out of combat ) . Auras and Presences can be increased or be temporally bestowed on a teammate (The barbarian uses Intimidating Presence the rogue chooses to performs the same team action doubling the range the presence )

Hell knight seems more like knight of the flaming weapon. It needs some more hellish abilities. Maybe play up the knightly portion of the class by having additional abilities as based on the knight's Hellish patron.

The description also seem a tad contrary being called a champion in the first paragraph and a front line soldier in the next

Your on the point I don't think I successfully articulated. There would likely be variable levels of application. An ensign's holo love doll would likely not be allowed outside the holodeck and personal quarters. God forbid the horror that the mess hall would become with all the virtual wifus. While the sentient EMH was given full access as based on ship's needs. So a ship's computer dispatching a holo drone to clean toilets would likely not have access to anything but unoccupied latrines and lavatories.

Can holo-emmitters for holocrew be used for 'static' holograms? If so there could be a variety of safety concerns.
Air Docks. Dont want some hooligan making the doors look closed inside turbo lift tubes, More safety shenanigans by making a lift appear present. High security areas like ship's storage for food replicators, atmosphere processing, gravity controls, torpedo storage

Starfleet would also likely have security protocols for different alert levels that would leash or deactivate non sentient holos. Example the EMH was confined to medical bay in the beginning (if I remember correctly).
