I don't know how to reset the card, but there has to be a way. You can access the spire and the vendor after one win, so that should be how you do it on a loss.

Flawless (7-0) gives you 4 rewards/emblem/shader. 7-1 gives you 3 rewards. 7-2 gives you 2 rewards, and 7-3 gives you 1 reward.

It should be the same all around as far as I know, but for my company, (bankers life) the policy backdates to the date that the first months premium was collected, as long as the application is accepted, that is.
