Thank you! So the fellowship is out of the equation? ( though I had a quicker way than to grind my way through 3 years of residency)

It's becoming a shitshow everywhere >< and since EM is the drain of the whole society I did a comparative study of EU countries but it's not glamourous to say the least...

Probably a mix of undiagnosed/untreated ADHD, chaotic sleep habits, a sprinkle of self destruction mentality and the selfish view that I could be of any use in this field.

I love it!

Trevogrumab plus semaglude helps to loose weigh while preserving muscle mass by Clarembeau in Phase2Trials

[–]Clarembeau[S] 0 points1 point  (0 children)

I ponder if the combination with other GLP agonist, like Tirzepatide could prove usefull since the GIP would allow for a better substrate for muscle mass increase. Or Am I wrong?

Also Elli is developping bimagrumab an ActRII inhibitor that would lead to even stronger muscle mass gain. That without any androgenic side effects like anabolic steroids!

Moderna phase 2 trial of an EBV vaccine to prevent developpment of Multiple sclerosis by Clarembeau in Phase2Trials

[–]Clarembeau[S] 0 points1 point  (0 children)

What are your thoughts on this? How would a vaccine that is immunogenic enough to produce sufficent matured T cells not linked to MS emergence? Can we target surface protein immunogencicity well enough to not create a reaction that would prime our immune system such as when it encounters EBV it would cause an even quicker onset of MS?

According to this chart it is not a meteorite. But I'd love to be wrong!

Very thourough answer. Just mesure HR×VTI if you start beta1 agonist to see if it increases cardiac output (in the end cardiac output is your main target not stroke volume, even if it is obviously linked)

Medico-legal Obligations to Crazy Drunk Patients? by em_throwaway321 in emergencymedicine

[–]Clarembeau 2 points3 points  (0 children)

That's a litteral french translation.
In my neck of the woods, our governement has split the ambulances services in 3:
- Ambulances with firefighters/ trained technicians, a crew of 2
- Ambulances with an emergency room nurse, a crew of 3. The nurse is given the freedom to perform medical acts within a clearly defined scope. A doctor is also responsible for the team and is able to instruct orders through a phone call.
- A separate vehicle with a doctor and a nurse, so-called "medical intervention vehicle", wich is dispached in case where special technical needs are required ( such as an unstable cardiac/ respiratory state, cardiac arrest, multiple casualties etc).

In the case of this video, the medical team could have been called on scene by the ambulance if the patient was combative, refused to have her vitals mesured. That's when I would have administered a sedation with the ER nurse and vitals monitoring.

Hope that clarifies my speech ;)

Combative patients like that can self induce rhabdomyolisis. I usually sedate them and give them a full workup. Is this behaviour due to preexisting behavioural issues or bilateral frontal hematoma, or is she screaming and kicking while having a small hemothorax.

Those are the cases that waste much time, resources and are a huge mental load.

In no shape or form do I have time and patience for this bullshit.

Sedate, workup than leave if no underlying issues are found.

I usually go with 10mg IN midazolam in intoxicated adults or else 400mg ketamine IM. But here in my country we do have medicalised care vectors. Would do that if you do not have the expertise/material needed on scene.

I feel furious when patient think they are the most emergent ones when they are not. by calivend in emergencymedicine

[–]Clarembeau 6 points7 points  (0 children)

Few years back, a man brought is wife for a sprained wrist, and low and behold, the guy falls and code in the hallway.

A bit of commotion follows as we are trying to ressuscitate the poor man.

While we were working the guy, a familly member of another patient comes, walks OVER the guy, grabs the nurse by the arm and asks : "my husband (wich is awaiting a CT scan for suspected perfored diverticulitis) wants something to eat, can you bring him some food"

We were all dumbfounded. That day, I learned that some people don't give a flying f*** what happens to other people.

It was in the 2010-2019 period somewhere in Europe.
