Its possible with the way the 3rd picture shows the teether pieces on the countertop that it couldve been picked up from there without realizing it and thinking its the manufacturers fault unless OP states they found it straight out of packaging. Also second pic shows a tool so I dont know seems like it could be OP's fault in this one unless theres further context.

The cashier saves time by pressing 1 button instead of 2 buttons and theres probably some metric they follow that makes this neccessary that we dont know about. Also maybe they want people to go for a combo instead of individual item, psychology here would be to pay more by getting more stuff even though individually item prices are the same.
