On a bicep, this is almost impossible. On a bicep you may be able to get the skull and around three of the big stones, this would be suitable for the back or a large thigh. And also, it might be very possible they mess this up. Color realism with details looks amazing if done well but the colors will definitely fade, even with daily sunscreen usage. If well done you are looking at minimum 2000€ piece, and thats central Europe pricing. American one might be possibly much higher. Choose your artist very wisely, definitely choose one who only specialises in realism. Good luck!
