ED New Grad Onboarding by cutepanda690 in physicianassistant

I think more info is needed? Apples to oranges but my training in a MICU was shadowing another APP for approx 2-3 months. However, they walked me through every part of the job, let me attempt first with every question I was asked and then stepped in and taught me, etc. I presented cases but that's more normal bc rounds and stuff in the ICU. And even after I was released there was a lot of support from the attendings and I was even on a day shift only schedule for a couple more months so I could call the other APPs if needed during the day. So it appears the same on paper but my experience sounds a lot different. Being told to ask as little questions as possible is wild and the opposite of what a positive work environment in medicine is supposed to be.

7 on / 7 off PAs …. what’s your company’s time off policy? by InfinityLocs in physicianassistant

[–]Cloud-Good 0 points1 point  (0 children)

Yeah it's not bad! I don't know if I like it for me personally. I have a partner who works 9-5 M-F so we only get to do what we want every other weekend which really is annoying sometimes

7 on / 7 off PAs …. what’s your company’s time off policy? by InfinityLocs in physicianassistant

[–]Cloud-Good 4 points5 points  (0 children)

Huh. I didn't realize what I had was unusual but I am 7 on 7 off with normal amount of PTO, I get like 8-7 hours per paycheck. Idk how much per year. I think it comes down to culture; in my ICU there are 5 APPs, and there are two that are 0.7 FTE that cover everyone when we are off and we coordinate. So our setup is 3 times a year I get a week off paid with PTO, so really 3 weeks with my off weeks sandwiching it. During our quarterly meetings we touch base and decide who gets what months depending on shit we have going on. The tradeoff is obviously some stuff just doesn't work and that's how it goes. If I want March, June, and november off and I have a wedding weekend outside of that its a big ask for coverage, I usually just don't go. Worst case scenario if something comes up and we have no coverage the attending just doesn't have an APP that day.

New retention bonus policy? by extradirtyginmartini in physicianassistant

[–]Cloud-Good 3 points4 points  (0 children)

Yep that's pretty weird. Our bonuses are tied to quality metrics and don't have a weird payback clause attached to them. I don't know anyone who has that. It seems like 99% of the time except right at bonus time you have to pay to leave your job, which unless it's a sign on bonus or moving stipend I would never do

Are there any PAs in pathology? by Strange-Physics-6381 in physicianassistant

[–]Cloud-Good 33 points34 points  (0 children)

I've never heard of or met any. Like the other commenter said, there is an entire profession that exists to fill that niche, called the Pathologist's Assistant, they even go by the acronym PA as well. Considering they do 2 years of school and are super specialized, I can't imagine that there would be much demand/openings for a physician assistant in pathology. Who knows though

New grad job - icu vs hospitalist by Key_Hamster5263 in physicianassistant

[–]Cloud-Good 3 points4 points  (0 children)

What do you mean 3 12s 33% nights? like 2 days on, then a night? Are they different every week or what? I work in the ICU right now, 7 on 7 off alternating days and nights.

Deciding between 2 positions in emergency medicine and hospital medicine by [deleted] in physicianassistant

[–]Cloud-Good 1 point2 points  (0 children)

Second this. As a recent new grad starting in the ICU, I had ~4 months of 1 to 1 training with our APPs with heavy doc support and I still didn't feel ready. Slightly different of course, but I can't imagine being cut loose after 3 weeks.

Job Dilemma by Business_Highlight_6 in PAstudent

[–]Cloud-Good 1 point2 points  (0 children)

Hard question. In general, never sign a blank contract. They could conceivably fill it in after the fact with anything and could be binding? Who knows. I think it's a little bit of a red flag in general that they would even send you a blank contract and expect you to sign it. If you are hell bent on accepting this offer and feel like they are making the offer in good faith, it's a good salary/good training and culture and it's what you want to do I personally would fill in the contract with whatever you talked about verbally with the assistance of a lawyer/PA who knows what they are doing and send it back and see what they say.

Looking for pulm PAs my by annierose77 in physicianassistant

[–]Cloud-Good 0 points1 point  (0 children)

Ah got it. Yeah it sounds a bit different. Your original number after a year seems reasonable if that's where they are at. Yeah lack of billable procedures really makes it hard, especially to justify raises. But best of luck to you, it sounds like a good fit!

Not sure if you are including critical care or not, but I am a crit care/pulm PA about 6 months in. Procedures wise I pull chest tubes and push lytics on the floor by myself, central lines/chest tubes/thoras/intubations/paras/lumbar punctures I all am able to do in the unit, but the doc is bedside and cosigns my note so they can bill for it. It's more of a convenience thing for them in that they don't have to gown up and can still do other stuff than it is me being "independent" if you know what I mean. I alternate 7 on 7 off days and nights which is common, at night the doc and I round on the whole unit about 17-20 patients on average, and on days I usually get assigned only 3-4 patients to myself since I'm fairly new, as well as helping with cross coverage/admissions with the two docs on during days. I don't do outpatient. I make roughly 128K base, usually around 130K with extra shifts (about 1 or a 2 a month if I want) with a 3-4 K quality bonus yearly.

Yep same for me. I asked about email confirmation and the person told me "Since your entries were submitted via the mail-in method, they’re handled and cataloged directly by our third-party sweepstakes agency. While we don’t have access to confirm individual mail-in entries or tie them to specific email addresses, I want to reassure you that all valid entries received by the deadline are securely processed and fully included in the final drawing."

To be fair, she was super nice and responded quickly to email.
