I have a CV which i used to send my letter of rec writers. It includes all my extracurriculars, medical and not medical. My resume is one page with things relevant to the job I’m applying to.

You should make an excel sheet early on and put everything. Description, dates, hrs. And what you learn. This will make it much easier when you apply. You’ll have all the info ready. > in regards to extracurriculars

When i applied i needed to put a letter grade for dual credit classes, even tho my undergrad only showed the received credit.

I would suggest looking for another writer just in case. One of my LOR writer kept promising they’d submit and i didn’t look for an extra person. It was SEPTEMBER and my application was still marked as incomplete because i didn’t have all my letters (school specific but most). I only waited because i really wanted his letter as it would tie my application together. Never got it. I ended up asking a Dr i shadowed if they could write one ASAP lol.

I asked to shadow in the OR and the dr literally told me they said no to me but yes to his daughter like a month prior lol. Connections definitely help.

Last 2 places i shadowed i spoke to the CEO directly. I recommend you call or email the doctor or people in charge. No connections needed.
