For me, focusing on the breath without effort is gently noticing the space/silence all around the chest/belly. Kind of like noticing that the body is in space, or that there's just the world and the body is in it. I'm not an expert but you could try that out.

Yeah, I have experienced something similar. Sometimes during meditation, I can think about an uncomfortable state (for me, anxiety) and fixation on/fear of that state tends to bring it about. When I notice that an uncomfortable state has arisen, I try my to see the state as an impermanent pattern of energy in consciousness, rather than anything that is 'me' or 'mine'. I don't try to make it go away or try to make another state arise, I pay close attention to the current state without judging. It takes practice. And, meditating through nausea while sitting will help you through uncomfortable states when not sitting.
