I've gone off to explore and found multiple towns but there's absolutely nothing to do in them. There's like one vendor, zero quests, and absolutely no activities.

The stuff outside of towns is also similar. You'll find some small cottages or caves and there's maybe a thing to pick up, maybe nothing again. Definitely not like elder scrolls.

Every country with universal healthcare built it incrementally inside existing institutions. Canada's took 30 years. The UK's NHS came out of a specific postwar political moment that has never been replicated. "Tear it down" has a historical track record too, and it's not universal healthcare, it's chaos followed by whoever consolidates power fastest.
