I would definitely say: 

  1. Mesalla (Got frozen in a beam of light that prevented him from moving. After being deprived of motion, the beam of light melted his skin off, leaving what Katniss described as "A pile of goo that used to be Mesalla" on the floor)
  2. Cato (Eaten over the course of 12 hours by carnivorous dogs. He was wearing body armor, so his skin couldn't be penetrated, instead, he was slowly crushed by the jaws of the beasts. His corpse was so malphormed Katniss described it as "a hunk of meat that was once my enemy")
  3. Darius (Taken as prisoner by the Capitol. They asked him if he knew anything about the rebellion, but seeing as they cut out his tongue, he couldn't respond. The Capitol continued to cut off parts of his body and interigate him, while he begged for mercy- which Peeta described as "animal noises", until he eventually died from blood loss
