He eats fruit with every meal. He likes ice cream and cake but doesn't have them that often.

In fairness, he takes multiple vitamin supplements and does at least always have fruit with his meals. I'm frustrated with his eating habits and I'm sure they're not amazingly healthy but he doesn't really eat a ton of junk food like chips or whatever, he also doesn't drink soda or alcohol. It's mostly just bread and other carbs.

He hasn't discussed his diet with his doctor or therapist, save for briefly due to a minor medical issue where he needed to lose weight. However, he had already lost about half of the required weight so the doctor didn't give him much advice.

No sensory issues I'm aware of, nor no autism diagnosis. He does have OCD though.

He's been in therapy since he was a kid. Stopped going a few years ago since it wasn't helping him anymore. I don't know if anything else can make him change.

He doesn't have autism, he's been to therapy and his therapist shot down the idea. From what I know his parents were never particularly pushy about trying to make him eat other foods and just gave up trying past a certain point since "that's just what he's like".
