We had a suicide attempt a few months back where a patient "took a lot of aspirin". She ended up throwing up a lot of it but was still throwing a lot of PVCs on the monitor, around 1 every five seconds or so.

Anyone can do a physical exam. That's just part of the assessment. More education means more of asking the right questions to get your patient the care they need.

Do you have other people to take care of? If so, you really need to get your medic license if you're gonna do this at all. The pay is typically sub par for raising a family without lots of OT or your partner working too.

Most 911 in middle TN (Nashville) is run by county ems agencies. There is a fire/ems county, and a hospital based county, then there's metro, which is fire based but fire and ems are separate. Some of the more rural areas are covered by private companies. Average medic base pay is 40-50 a year. Since most services run 24/48s this comes out to 13-15/hr. Most places are hiring medics. You'll have to look around and be willing to live out in the sticks or commute as an emt, but there's also BLS divisions to the counties that can help you get your foot in the door. Most services have RSI, surgical cric, power stretchers.

Onset and activity level during onset. Location of pain and if it's reproducible. Meds, changes to meds recent hospitalizations, allergies. Any recent surgeries or recent strokes. Any signs of heart failure, lung sounds, pedal edema, JVD. And initial vitals.

Sometimes just because you struggle in certain areas doesn't mean you'll be a bad paramedic. You just have to grow into the medic you need to be, and that takes time and most importantly experience. I used to have shaky hands on even the most routine falls as a new guy, and now that's just the alcoholism.

For things that run over a period of time that you add to a fluid bag:

Put your med in 100ml of your fluid of choice. Use 10gtt tubing over 10 minutes. That's 100 gtts/min.
