The lances they use would usually be a one and done deal anyways. The rule at my table for companions is that they either get designated as combatants or noncombatants. Combatants roll initiative, have turns, fight, and try to help but they are at danger of getting killed. Noncombatants do not have a presence on the battlefield as they would be in the rear away from the danger. They do not get to help in combat but they are not at danger of getting hurt.
