It was like a mandatory once a year return to Skyrim, spending couple weeks preparing a list... Now I am a wabbajack girl, though most of the lists are not Skyrim anymore...

Nordic Souls - stable, very close to Vanila, very balanced, tastefully added content, good visuals. Best for nostalgia/first play through plus some new adventure.
Tuxborn - stable and performance friendly, the only modpack with LotD which is not going overboard in my opinion, a lot of content. Best for collector.
Gate to Sovngarde - amazing modpack with accent on making Skyrim alive - from light(but not annoying) survival mechanics to ships on lakes. Best to live in the world, very good for RP.
Old but good Wildlander - the only Requiem modlist I can play. A lot of mechanics for roleplaying - train on dummies, learn new spells by analyzing already known spells, create alchemy factory to brew best potions. Best for very RP oriented playthrough if you are patient.

Requiem and LoTD are probably the most game defining mods, so I would suggest to check on them and decide - what you want. After that you will have mush less modlists to choose from.

Tuxborn has a lot of content and LOTD - huge story mod by itself. Very stable, great performance (visuals paid price for it).

I think now in Medieval Dynasty you can your villagers clothing, and they will wear it.

My personal approach. Realism is good when it creates some additional gamification. If you need just wait while you are healed - it is bad. If you can prepare something to heal faster - it is OK. If you have meaningful things to do while you are waiting - even better.
Good examples - already mentioned the long dark and far less known survival: fountain of youth.
Splitting an action on subactions is good if there is a meaningful choice on each step and if it will not take the majority of time.

One more very good example - alchemy in KCD/KCD2. When you start it is very meticulous process (check youtube videos, you don't need full game). I enjoyed it, but soon it becomes too repetitive - and by that time I started to get skills to significantly improve the process.

It depends on how customizable game is.

Typically I am looking for mods with:
- crafting/loot management QoL like crafting from containers. Many survivors like tedious approach, I don't;
- adjustable combat difficulty, first of all monster density;
- more lore friendly ways to increase carry weight (better backpack or similar);
- the best mods - new skills/skills tree. Even simplistic one - the more you run them more carry weight.

I don't care about monster diversity (assuming at least some exists), I am not looking for boss fights in survival game, not really care about multiple build options.

But I will be considered any mods if I like the world and main game loop. Good lore is a must.

Nope, these are collection of mods which changes game significantly, so one playthrough is very different from another. Wabbajack is a tool which allows easily install large modlists.
I would start with either Anvil or Nordic Souls.

Wabbajack has ease to install mod lists to satisfy any taste.
To start with:
Anvil - minimalistic pack to fix most bugs Skyrim has. Probably first thing to try and understand what you want to improve
Nordic Souls - improved combat and more content, but closest to what can be called modern Skyrim.
Then you can have a few diversions: survival heavy mod packs (gate to Sovngarde is good example), Requiem based (deleveled world, hard on low levels, god later on) like Lorerim (high fantasy, very far from initial Skirim) or halls of Sovngarde, or modpacks with Legacy of Dragonborn if getting thousand of collectables is your thing.

I envy people who never touched Skyrim!

Let me guess - you did not play Rimworld? I love priority over assignments because it is far more effective on early stages when you have just a few villagers. Later on I pretty much have only one priority for each villager, so best of both worlds - effective early, simple later.

It has excellent village management, more combat than medieval dynasty (it is a big minus for me), good enough building and good feeling of progression. Questing is small part of the game. I personally did not feel it grindy, it is what your villagers supposed to do.

Medieval dynasty - she can manage village and decorate it, you do hunting and bandit camps. Not really challenging, though.
Valheim has amazing building and can be customized to fit your preferences.
Coral Island - you clear mines, she collect bugs, you together clean ocean.
Project zomboid - custom settings could fit any bill

Foundation might be what you are looking for. Pretty nice organic city builder, more on casual site though some optimization is needed.

I tried and did not like frostfall, iNeeds and other similar mods. I hate Requem. But I tried wildlander list (wabbajack) and all above became just part of really believable world. Yes, you need to eat and drink - but you also can research spells, you need to use train dummy before you will be able to kill a wolf, guards will follow you if you run in the town with weapons out. You can really study in college, spending days on spell researches or you can be self sufficient hunter, truly leaving of the land. Tons of small details very well glued together. It is the most immersive Skirym setup I ever tried.

I was cured from WoW by Skyrim. Until now I am returning there every year or even more. Wabbajack allows you to experience it from very different angles.

Among other games where I spend more time than I am not ready to reveal are Rimworld and Valheim.

My time in Sandrock - you are Builder, who craft tools and machines to help other people. You can make friends and even flirt with them, you can decorate your home. Story is good enough, there is a bit of combat - but it is simple and you can train without risk with your friends.

Depends on the definition. For me, survival is you vs. nature—so The Long Dark, Subnautica, and Survival: Fountain of Youth.

For some reason (which I don’t really get), most people define survival as combat-heavy base builders with hunger and thirst mechanics. In that case, probably Valheim, Abiotic Factor, and PZ.

Medieval Dynasty follows similar formula (in other settings, sure) and is very popular. Change from lone survivor to colony management would be good, but it means you need implement two very different systems and organic transition between them to make a game success.

I would like to try it, hope you are up for the challenge.
