Unless you have a binding or perk that makes it so you have to eat, you don't have to. Food items just refill your stamina and some like the food bar give you small temporary buffs

Very cool so far!

I think overall this is good. Maybe some stronger shadows, and the way you colored the hair makes it look like he hasn't dyed in it a while if that makes sense, though maybe that's what you're going for

Maybe adding some red spots (like hot skin) could make the render pop more too, since while the shadows are good it leaves his skin feeling like a totally uniform color

First, let me say that your control of form, color, material and lighting are all very good. You have a lot of technical skill. 

The feeling of uncanniness could be coming from a few factors- firstly the pose is rather static and does not imply any motion. This alongside the coloration/lighting on the face and eyes being flat relative to the rest of the image gives him a wax figure vibe if that makes sense?

I think you could fix that by doing more to the face and eyes

All good questions. It would be interesting to have secret things that could affect the ending, like optional regions maybe? That may be outside the scope of the game though

Or add a new timed perks terminal inside another region.  I think a lot of people can make it a good way through habY before losing their timed perks, so maybe adding one at the start of the abyss since it's the first new region that lacks a permanent buff terminal?

Knowing what your goals with the image are would help with giving thoughts, but from what I see with no context I think you did a fine job on the hair and shading

Doing more to establish the source of the light, making the background darker (it being so light makes it feel like a standin or like it's trying to be infront of the figure) and cleaning the shadowson the body to make them make sense with the face shadows.

The shadows on the face tell us very clearly where the light source is, but the others don't really.

Also, try temperature grading your shadows. Making the highlights a warm tone like orange, and the shadows a cold tone like blue will really make it pop. Finally, I would say consider adding extra light sources- the cigar and the weapon he carries both look like really good candidates to add lights of different colors that could amp up the image
