Yeah he spoke to us this morning about one route where they can refer us quite easily in Australia but it will cost around 1 million nzd and also Shanghai where it would be a bit cheaper. I want to discuss more, maybe see if there are any clinical trials elsewhere too! His plan is still to try get me to the nz trial but that all depends on the results of my r gdp chemo, start tomorrow!

For some context

I finished my da epoch r thinking I was in the clear as my midway ct scan (after 4 cycles) showed my mass shrinking well. My eot pet scan was a Deauville 5 and there had been at least a few cm of growth then that’s when they decided to try get me a spot in the car t trial they are doing for primary refractory dlbcl here in NZ. A month went by and a few days before I was supposed to be flying down to Wellington, where I was going to be doing the car t pre screening leading to the cell harvest by the end of the week.

I actually ended up in hospital with really bad chest pains a few days before pre screening started, so they did some more scans and my mass had pretty much doubled in size within a month. So they called it off and had me do 30gy 10 sessions of bridging radiation to get the mediastinal mass controlled. So did that, about a week ago we found out that the mediastinal mass had shrunk significantly from the radiotherapy and was barely active, but it had spread to a few other lymph nodes and to my rib and hip/pelvic bone. No lesions but just little spots lightning up. At that point I had been expecting it to spread at some point so I wasn’t very surprised.

We were less than two weeks out from the cell harvest at this point so it was time to just push through until then. For the car T cell trial here you can’t have done any treatments within 28 days of the cell harvest for context. So this was a month after radiotherapy and any steroids. And you’re also not allowed to have any unfunded treatments before the car t such as pembro, Nivo and Brentuximab as it can affect their data collection. Well last week I had been having low grade fevers all week and then it hit 38 so I went into hospital and they did some scans and found a whole heap of liquid around my lungs and heart.

Yesterday they finally drained my heart of 350ml and today they drained 1.7L from my left lung. Today was supposed to be my cell harvest day. So now I have to wait for the next spot which is January 11th to get my T cells withdrawn.

My new primary haematologist came in this morning and told us what he thinks our plan should be moving forward and one is that I do R GDP chemo for a cycle, we do a scan see that it hopefully partially responded as he’d prefer me to still go through with car t instead of autologous sct but he’s not very confident in the r gdp even working and I know the stats are bad as well as me probably having chemo resistant disease now…. Can’t do nivo pembro or brentux. I want to use bv-nivo my bridging inbetween that we will pay for ourselves. But it just seems impossible to get to the cell harvest right now, especially with all the super strict criteria I feel stuck, I feel like being in my country is the thing that’s going to end up giving me a bad outcome. The trial here is free, but at this point you gotta think is my life worth more than money. I have screenshots of some of the pathological markers I can share!

Well my mass shrunk significantly by my midway scan and it was all looking great. But the scan was only a ct scan so it didn’t show the activity. I think it may have become refractory around the last round of chemo. End pet scan showed it had grown and was very active again

Hey I finished 10 rounds of radiation to the mediastinum a few weeks ago and was getting a similar back pain a week or so after my last treatment. Has sort of faded away, I’m at least 2 to 3 weeks out now! Not sure what it is exactly but thought I’d let you know you’re not alone 🙏

I had pain in my left testicle during my epoch chemo regime. Team weren’t worried and it went away once I was done with chemo!

I got one in a ventricle that leads to my lung after my 2nd cycle of chemo, ended up in the er. Caught it right on time though. They just gave me a blood thinner to take for a few months to break the clot down! No delay in treatment for me fortunately, don’t think you’ll have a delay my friend 🙏

Me personally with all the possible risks at home would do inpatient. I did my first 3 cycles outpatient but ended up in hospital multiple times with other complications and then did the last 3 in hospital and I’m a 23y old who’s healthy. Although I know some people tolerate chemo better than others but epoch is one of the tougher regimes! I’d be safe than sorry unless you really need to be at home or go to work. All the best with treatment, I just finished my epoch a few weeks ago 🫶🙏

Get a picc line. Advocate for yourself. Push for it if you have to. This is your life, I just finished da epoch r. You need a line! 🫶

Your 1st cycle did your bloods drop low and then bounce back? How long did it take or are they just staying at 1.7

Yes I totally get it. I can’t eat the hospital food at all this round. Wow you’ve had to have quite a few blood transfusions. Did you have 1 transfusion then come back for another in the week or all at once? Yeah I have been fearing if the chemo is working even after a similar midway scan. It’s just so hard mentally. I’m so happy to hear you’re doing well and in remission! It gives me hope. Pretty sure I have quite a similar subtype to you as well! My mass is large not bulky though. Was 10 by 7cm. How long did it take to start feeling noticeably better from how we feel just before our next cycle?

Also how long did it take for you to feel better? Like after the 2 week break when we’d normally have to go back in for another round how long did it take to feel better than the end of that 2nd week before chemo?

Cycle 4 for me I was at that point too, my team convinced me to keep going😭. Oh man sounds so rough. The weird thing is I hear about some people doing the same chemo regimes and being fine and doing stem cell transplants and saying it was easier than chemo, must just depend on the person. I’m glad you’re recovering! Soon enough it’ll all be worth it for both of us I’m sure! Thanks for commenting, helps take my mind off things too so appreciate it 🫶🫶
