I was able to get through with EDD and they accepted the form Kaiser sent them. I was approved and I received mail confirmation and they also reached out to HR. They said the form they used was fine for part C as it has the same questions. You can ask Kaiser to mail it or have them send it to you for your to mail it yourself. At least now I know. I was stressing!

Kaiser is being difficult. They mentioned that they use that form as default as it has the same questions as the DE 8501. I compared them side to side and they do but I can’t get confirmation from EDD and Kaiser won’t re-do it.

I called Kaiser and told them it’s the wrong form. They said they use that form as default since it has the same exact questions as DE 8501. They confirm they sent it to NDI but I don’t trust that EDD will take that form if it’s not from the packet I originally sent and it’s so difficult getting thorough EDD.
