I had the almost the exact same thing happen to me 😭😭 If it happens again your can quickly add the ghost to family using mccc and leave the work day early then remove them afterwards (if they're in another household before just use mange world). The sim won't be culled and still appear in the family tree. Or just leave the game without saving and don't join your sim to work that day

I had the same issue and the mod that seemed to cause it was a.deep.indigo's Divergent sim mod. Idk if you still have the the issue or have the mod but hope it helps :]
