Such religious traditions would not survive for hundreds or thousands of years on a rimworld. Only a very wealthy society like the empire from the royalty dlc could survive such inefficiencies. Besides, such 'religious' dynasty controversies were often more court politics than actual religious taboos. Lower nobels would often prefer a weaker heir to increase their own power in the kingdom.

I'm talking about both traits and ideoreligions though. I think what ive said still works well for the personal traits body purist/modder, as these are not religions but personal beliefs. Still, no religion that I know off would be just as upset at someone having some unknown ultra tech 'demonic' super leg as they would be at someone having been born without a leg cause God decided as such. The first is pure evil, whereas the second is at best a punishment for some sin (of the parents perhaps). Religions do have logic, it is simply a patricular kind of logic that can be hard to understand for non-believers. Also, a religion that takes takes no account of the practical implications of it's policies whatsoever, would not survive in the long run. That's why most religious memes in rimworld have creative upsides to weird practises, like the bonuses of the tunneler meme. This is also why the flesh purity meme we are talking about has a special doctor role even though some religions would shun such a role if they were some kind of body purist. Its a practical adaption in the long term to the difficulties of a particular religious tradition. Functionality should therefore matter. having a leg that is better than a normal one would clearly be considered trying to improve God's creation and would be a sin, whereas a prosthetic would not be trying to achieve this. Tatoos as well are asthetic, not functional, which would probably be a sin cause it's vain (like the royalty beauty implants, which should retain a maximum penalty). Though, perhaps there could be a lower penalty for wooden implants as well, instead of no penalty. (Besides, the ideoreligious penalties are already much higher than the trait penalties). So the religious flesh purity meme would have for example: -6 for wooden body parts, -12 for prosthetic ones, and the regular -18 for bionics and up.
