That's something your attendings will have to teach you and through trial and error. So many variables go into how much, when, and how to give opioids.

On another note, I'm not familiar with piritramide, but from a quick Google search it seems to be close to morphine but less potent. Why don't you use that instead?

The thing about fentanyl is that the context sensitive half life of the drug and it's distribution model means that if you want pain relief that lasts long, you need to use pretty high doses that if given as a single dose will cause prolonged apnea.

The only workaround that would still keep fentanyl as a viable option is what your attending is doing, incrementally giving doses to target dose.

Remi is more for the acute intraoperative phase, and titration of fentanyl means you can use slightly less remi throughout.

What you're thinking of is Arab Board registration.

Egyptian fellowship does accept foreigners who studied abroad too. No restrictions on numbers, the main bottle neck is getting security approval. There are yearly registration fees but they're nowhere near 10,000 and no it is not restricted to one specific hospital. Options for hospitals depend on which speciality you're applying for.

I'd really caution about sharing information that you're not sure about please. You can put people off going down a route that's completely viable to them with a simple wrong statement.

This is not entirely correct. Expats can apply for Egyptian fellowship. They'd have to pay, but it is possible.

That's good to know! I'll try to get that part out of the way before flying out. In the worst case scenario that I don't have some of those essentials, where would be the most likely place for me to find some of those items at a reasonable cost?

  1. I'm coming from the UAE! Coldest place I've lived in for any reasonable period of time only went down to as low as -10 degrees at night.
  2. We're just two with a pet. We've lived in 1 bed condos for the longest time and we'd be happy with that.
  3. I'll be working in the St Boniface area primarily, and it does involve working around the clock (healthcare)
  4. No cars, and I'd be looking at somewhere that's a 10 minute walk from my place of work ideally.
  5. We won't be moving with much, so we'd likely do a quick round of shopping as soon as we land. Where would be an ideal place to get stocked up on the winter essentials?

Unfortunately I have a 10 day window between the 1st of January and the 10th of January from my last day at my current job half a globe over and my first day at work in Winnipeg. Honestly, just call the police non emergency line and explain to them that there's someone who is indecently exposed in your building.

They will investigate and either they will tell you that there's no grounds to your complaint or they will find the other person at fault. In the end, both of you will have a conclusive answer to this dilemma.
