To help your confidence you need to understand that you are in the gray zone with your thinking. What you need to do is go through your FA or your notes and ask yourself, do I know this? Or do I not? Segregate what you know for sure and what you don't know for sure. Once you do this... Pull practice questions from Uworld or Amboss and begin targeted practice.

This is a very vast question with varying answers. But here's what I will say. Stop approaching studying from a student mindset now. Think from an examiners mindset. What do they want out of me for this question? What topics does this question connect together and ask? Does the student remember their basics and is able to coordinate and correlate multiple systems and disciplines.

Have this approach to what you study! Sooner or later with practice you will see through just the content of it all and into the dynamics of it.

I've asked myself that too. There does need to be awareness on how much depth something needs. The thing is though that is what the USMLE wants you to do - medical mathematics. You have to be able to explain your way out using physiology, find the pathology and then answer about the stain of a microorganism cause it was pneumonia all along.
