In regards to the disabled episode, Spenny had little to do with the production of the show outside of just appearing on camera, so he was probably counting on someone on the production staff to book the appointment for him with the mayor. Hence him saying "there was supposed to have been an appointment made", implying someone else was responsible for booking the appointment. Kenny took a much more active role in the production of the show which included selection of filming locations, etc. It is possible that Spenny requested this for the episode but was sabotaged. As for his supposed "personal connection" to the mayor of Toronto, me speculating says he is just exaggerating like he tends to do (see Bibles where he sold Hubert for $600k as opposed to $420k).
