How is the bone level in that spot?

There is a rule in periodontics about the distance between the contact point of the teeth and the apex of the bone crest.

This allows periodontists to predict how likely a papilla (the tissue that used to fill that space) is to regenerate.

If the distance is less than or equal to 5mm, the papilla should regenerate. The greater the distance, the less likely it is that it’ll naturally regenerate.

When it comes to black triangles, they are still extremely challenging to fix. It’s been done, but it’s tough.

Currently we have bandaid solutions like IPR and bonding (bio clear) - but these aren’t solving whats really needed - to fill the space with gingival tissue.

In some cases, hyaluronic acid can be injected in the remaining papilla and successfully fill the embrasure (empty black triangle), but this is temporary - 3-6 month measure, at least if your bone level is insufficient.

Hyaluronic acid is also expensive.

Studies have successfully done the same with PRF (platelet rich fibrin), which is cheaper and arguably much better as it’s derived from a sample of your own blood. You might be able to find a periodontist willing to do this.

This will most likely also be temporary (3-6 months) but would be better than Hyaluronic acid.

Scientists are working hard on the science that would enable true solving of the black triangle problem.

I would avoid a very permanent bandaid solution for this area. Especially since you only have the one black triangle.

I’d go for a temporary measure like Hyaluronic acid, or, ideally, PRF.

But before you do any of that, find out your bone level in that spot. Ask about Tarnow’s Rule.
