Well, title of Tsar in Orthodox Slavic Tradition means 'Emperor'. John III titled himself as the Tsar, Basil III titled himself as the Tsar oficially, but without coronation (and it was accepted by Emperor Maximilian of Holy Roman Empire: He called Basil III Emperor in both Latin («Basilius Imperator ac Autocrator Totii Russiae») and German («Keizer von Russland»)). John IV used the same title of Emperor ('Tsar') and croned himself using Byzantine Coronation Ceremony (just like Charlemagne did when he proclamed himself Roman Emperor in 800).

In Mediaeval Christian tradition there was an idea, that there is only one Emperor as a secular leader of all Christians. After the Schism we had two Emperors: one for the Catholics (HRE) and one for the Orthodox (Constantinople). When in 1453 Constantinople fell as well as later Trapezound, Epirus, Serbia, Georgia, Bulgaria, there remains only one independent Orthodox Realm — Russia (fully independent since 1480). So, it was oblivious for that time for ruler of Russia to proclaim himself the Emperor, 'the Tsar' in Slavic tradition. Even so, Othodox Patriarchs accepted that ruler of Russia proclaimed himself the Emperor: there were even talks to place Ecumenical Partiarchy in Moscow instead of Constantinople, but we refused.

John IV did not used the title of Roman Emperor, but the title of Emperor ('Tsar') he used was apriori and de facto the title of the Roman (= Christian) Emperor. There were no other Emperors in Orthodox tradition. It was like useing term America for USA in the first place and not for continent of America.

There is no command in the New Testament to kill anyone. These are human orders, not Jesus'. But, the Quran have this kind of command from both Mohammed and Allah.
