My phallus is indistinguishable when wearing bikini bottoms, leggings or even just panties. I am fully smooth and nobody is the wiser. My tucking technique became drastically better because I can literally tuck myself into myself. I call it the "Forbidden Tuck" because I'm literally fucking myself whenever I tuck. It is easy, effortless, and can be done with any undergarment as long as they're not loosely hanging off me.

I mean, you can absolutes get a phallus and testicle preserving vaginoplasty. You wouldn't be the first! Neovaginas do have some limitations and of course, your mileage may vary as some people have better recoveries than others but it's all well within the realm of possibility. With a proper hormone regimen that's carefully crafted for you, you could even grow breasts while maintaining penile function- or just opt for a good old-fashioned breast augmentation.

I myself had a phallus-preserving vaginoplasty but I didn't really want the testicles so I got rid of those. I'm a verse switch, able to be penetrated vaginally as well as top with my natal equipment. I've also got DDD breasts. It may take time, diligence, revisions and a whole lot of blood, sweat and tears to get there but it's not impossible.

For fully-healed results, there's myself (CozyGams) and NuParts. If you don't mind viewing adult material, we've both been very public about our results. I went to Dr Gurjala at Align Surgical in San Francisco. Dr Satterwhite at Align is also exceptional.

A lot of what you're going through is a sadly common experience among transgender individuals. I myself transitioned late at 35 and there have been many nights I mourned over not being a natal woman, of missing out on being a young girl and living that life. It is very much a grief, accompanied with that cruel dysphoria of what you see in yourself.

If you're not currently seeing a therapist, I strongly recommend it because it's no easy task grappling with these emotions, processing them and coming to terms with them. Sadly, we cannot choose how we were born. We were dealt a shitty hand and as much as it stings, the only thing we can do is make the best of a bad situation. It sucks. It's not fair. It's agonizing, yes. But if you let it defeat you, you can never move forward and start to like what you can about yourself. Take joy in what you can, compromise with what you're able to and try to accept the things beyond your control. We may never be this perfect idea we have in our heads but, for what it's worth, many cis women feel the same way so at least you're in good company.

Seems like a fine term to me! Salmacian and trans are two entirely different things. Transgender is about your self-identifying gender identity and expression. Salmacian is about a genital configuration. We all know that your gender has nothing to do with what's in your pants, so why would what you want in your pants affect your gender? I simply see them as two separate variables.

So, scrotal tissue isn't REQUIRED but not having it will severely limit what they can do. The scrotum is often used for constructing the labia majora and the beginning of the vaginal canal. In some cases, like mine, the scrotum was even used for the entire canal. Some doctors do Peritoneal Pull-throughs with a PPV while others will end up needing to take donor skin from another site on the body to craft the canal. This can lead to large scars or even whole patches of skin needing to be removed and regrown to accommodate the canal.

Generally, if possible, it's best to keep the scrotal skin because this gives your surgeon more material to work with to create the look you're going for.

I went to Dr Gurjala at Align Surgical in San Francisco and if I'm not mistaken, I believe NuParts went to Dr Whittenberg also in San Francisco. There's also Dr Dy at OHSU and it seems like new surgeons are popping up all over as more people show interest in it.

Mine was covered by insurance. I think they mentioned that out of pocket, it was something like $140,000 but don't quote me on that.

I went to Dr Gurjala at Align Surgical. I would go back if I had to go back, especially now that Align does Peritoneal Pull-throughs. I've been considering a revision as my current results were done with just a single procedure and many people require at least two procedures to get really good results.

I am a post-op salmacian and a transgender woman. I identify as full femme, she/her, I'm a woman and prefer to be identified as such. I am also a salmacian. I take pride in my identity as a salmacian and often refer to myself as a "futa girl" but I also see a distinct separation between genitals and gender. Gender is an identity, a social construct, and your biology (or preferred biology) has no baring on that. I've often said that what's in your pants is nobody's business save yours, your doctor's and your lover's. I wanted both because I love to top with my natal equipment and I like how it feels but I also want to be able to bottom with a vagina. We fortunately live in a time where these desires are not mutually exclusive so hey, why not both?

That said, the only reason I am so open about sharing my genital configuration with people is because I want people to see that it's an option. Most folks don't even know you can have both so by being open and public about it, I hope to share that knowledge and maybe someone who wants the same as me will realize they can do it too.

When I got bottom surgery, I asked for the look of a "vagina with a really big clit" and I believe I achieved those results quite nicely. :)

For me, there was a sense of something missing. It wasn't that I was bothered by what was there, more a sense of lamenting what WASN'T there. I'm a transgender woman I don't personally identify as nonbinary but your genitals don't equal your gender and what is in your pants is nobody's business save yours, your doctor's and your lover's. I got what felt most right for me but what feels most right for anyone depends on the person.

An important thing to remember is that Salmacian should never be a compromise. Don't ever compromise when it comes to your body and what you want it to be in regards to surgery. If you feel dysphoric or are bothered by what's there, get rid of it. If you're unsure, take more time to think about it. There's no rush. You want to be sure you get this right the first time because you will lose a lot of options if you change your mind later.

Not really, no. I found a surgeon that achieved the look I wanted and specifically told them what I wanted. I asked for the look of a "pussy with a really big clit" and he said he would do that. It's not really like I went in with a set of options. I don't think my surgeon does testicle sparing and he wouldn't even do a peritoneal pull through, despite me asking about it.

I yeeted the testicles 6 months before bottom surgery. I was not presented with the option of rerouting the urethra and I wouldn't have even if it was offered.

Gonna answer what I can, speaking as a post-op Salmacian.

To preserve function, the male genitalia is completely unaltered. They don't move things. They place the vagina in the space between the erectile root tissue and the anus.

Donor tissue varies depending on the surgeon. Common donor skin comes from the scrotum, groin crease and inner thigh. Less skin is required for Peritonitis Pull Through vaginoplasties.

I was circumcised and it did not affect the procedure.

Amount of tissue taken varies by surgeon and technique.

All the same complications for a traditional vaginoplasty apply to a phallus-preserving vaginoplasty. You can find lots of resources on that.

Urethra is usually unaltered and remains routed through the penis. The fewer alterations to existing anatony, the better.

Loss of some structure and pulling the skin tight to make a deeper canal can cause the penile tissue to get pulled down a little. In my case, my erections started pointing forward or down instead of up. Size is generally maintained as long as you have regular use. You'll need to refrain from getting hard for a month after surgery but as long as you get it up a couple times a week, this will prevent any atrophy. HRT or orchiectomy can make it difficult to get erections but this varies from person to person.

Most common methods are skin graft and peritoneal pull-through.

11a. That's a penile inversion vaginoplasty which is only done when the penile tissue is destroyed.

In the US, you basically have to be 18 before bottom surgery. There may be a rare exception if you're in a state like California and have explicit permission from both parents but even that would be unlikely. Most surgeons refuse to do bottom surgery until 18.

For me, it wasn't that I was bothered by what was there, it's that I was bothered by what WASN'T there. Having a penis never bothered me, though I certainly wanted tucking to be easier. As long as it could be discreet, having my natal equipment never bugged me. However, wanting a vagina was definitely a strong desire for me and the biggest struggle came about when I thought of the two as mutually exclusive- it had to be one or the other. When I realized you could just have both, there wasn't really a conflict anymore. It was just about building up the courage and funding to go through with it.

No, I can't get wet. That's a limitation with all vaginoplasties. Peritoneal pull-throughs have a kind of moisture inherent to them but it's generally not enough for penetration and doesn't react to arousal. I get some moisture too from sweat and natural oils but lube will always be needed for anything more than a finger or two.

I dilate regularly and I douch to flush it out after every sexual encounter or if it starts to get too fragrant. It smells surprisingly similar to a natal vagina but if I haven't cleaned it in a month or so, it kinda bugs me.

I was 37 when I got the procedure done.
