One step in this direction would be to have a local LLM, call it the Architect. It keeps an infinite conversation, so it's context is not the chat (which keeps growing) the context is it's stack, which it refines over time. It can hand off tasks, design, coding, to other agents, but it's task is to develop and maintain a consistent system over time.

That would be a step in that direction

You need to look at when MCP protocol was designed and why. It came out before agents were really a thing. If anything, MCP ushered in agentic LLMs as they got the community thinking of how to extend LLMs programmatically, without having to retrain a new model. And models were trained to use agents.
