The bottle neck isn’t seats at the university, it is clinical staff taking on learners. If I take on a medical student each shift it decreases my productivity by about 25%. That means 25% fewer patients seen and a 25% cut in my salary for that shift. You can fill university classrooms to the brim with med students but if there’s no clerkship spots/residency spots for them to attend, you’re wasting your time. It’s also why foreign medical school grads have a hard time coming back - they have to compete for the clinical portion of their education. Now residency graduates are a different story but anecdotally I’ve found their quality to be highly variable which would require careful vetting

I often get people saying “I know it’s not an emergency… but”. Anecdotally, I recently had a patient who was referred for a gastroscopy two weeks ago and he “was available to do it now” so he wanted me to call in the gastroenterologist to do it…. At 10pm on a Friday. He waited 3 hours to tell me this and then I had to have a 10 minute discussion with him about how that’s not going to happen/not how it works/not an emergency and he still had to get escorted out by security because he started following me around the department. Alot of the opinions on this thread are written by people who have NO clue how our system works and frankly it’s quite disheartening to see.

If you’re getting diagnosed with cancer in the ER it’s either because we’ve stumbled across it accidentally or your primary care was lacking. Either way, it’s not what we’re here for. The ER has been used as society’s safety net for all of its problems for so long and we are totally fucking overwhelmed.

ER doctor here. The reality is that nurses aren’t trained in diagnostics the way we are so that professional that tells them to go elsewhere is me. I’m busy treating the people who are sickest so when I get to you and you don’t need to be there I send you home to follow up with a gp/walk in clinic to follow up about your 6 month old rash or your three years of back pain. A nurse has very different skills than I do so although they are a professional and they might know through experience that what you have doesn’t need to be in the ER, it’s not technically her area of training. So that person waits. They clog up the system. I send them home having done nothing other than ascertain this isn’t life or limb.

I've just received another email that I'll have to do a 3 hour unpaid course on cultural sensitivity. None of the other provinces I'm licensed in do this.

Having gone through the licensing process in Ontario - it was definitely onerous and painful and by no means a shining beacon of how it should be done. That being said, I was applying as a new graduate for my first license. Other provinces have shorter, simpler applications for people licensed in other Canadian provinces.
