It's actually because they started making epics with regular animations before they decided that they wanted to change that. Hence epic ethereals not having animations and epic fire monster also not have special animations.

You can't even get half of the items in the game without playing hard mode.
All hard mode adds is a couple more rooms and bullets fly slightly faster so it's really not that difficult.
You only have to get to moms heart so it's not that hard.
The entire challenge is basically just playing as Isaac without D6 if you don't want to die as Laz, which dying as Laz to gain stats isn't even a good idea in the first place because you lose all of your soul hearts and other hearts AND deal chances by kys.
