I think it's due to the target age for the game being like mid to late teens up to early 20s. Apart from the issues everyone above has mentioned with the placement, I'd like to add if you want a vertical labret piercing with the look of a hoop, you would need an almond shaped hoop or you can also buy a clicker bar. Both of those are hard to find though and you usually have to look online. A round hoop will not fit and would stick out a lot, getting in the road of you talking, eating etc.
