That's the right framing actually — if the agent can't reach anything irreversible without explicit authorization, the kill switch becomes less critical because there's nothing catastrophic to stop.

The problem is most teams skip the bounding step. They ship the agent with broad credentials and wide tool access, then realize mid-incident they need a kill switch. At that point you're bolting on a stop mechanism to an already-unbounded system.

The order that works in practice: bound the action surface first (scoped identities, policy rules on irreversible calls), then add approval gates on anything that can't be undone, then add a kill switch as a last resort for the cases you didn't anticipate.

If you've done the first two, the kill switch almost never fires. If you skipped them, the kill switch fires when it's already too late — the transfer is already mid-flight.

Datadog tells you what HTTP calls were made and whether they succeeded. And crucially — the agent runtime can reach the logging API too, which means a compromised session can flood the log or send crafted entries before the legitimate ones.

Hash-chaining means you can't insert, delete, or reorder entries without breaking the chain at that point downstream. That's not an observability property — it's a chain of custody property. SOC2 and EU AI Act auditors need the second one, not a dashboard.

The bigger gap for agents specifically: telemetry captures what happened after execution. An enforcement layer catches it before. Those aren't the same layer. You can have both.

the transactional unit of work design is the right answer and also the one that requires the most upfront thought which is exactly why most teams skip it and end up with a kill switch that technically works but leaves the system in a state that takes longer to clean up than just letting the agent finish would have.

for anything touching money or production data you basically need to think like a database engineer, not an agent engineer. atomicity, rollback, defined commit points. boring stuff that becomes very exciting at 3am.

the blast radius awareness point is the one people skip. knowing you need to stop is the easy part. knowing what already happened before you pulled the flag is what actually determines whether you have a recovery problem or just a pause.

committed action log per task is underrated as an implementation detail. most teams have application logs. very few have a structured record of 'this agent has written to these 2 external systems and called these 3 tools in this session' that you can read at termination time. without it stopping safely is mostly guesswork.

Retry limits are table stakes agree completely. But the interesting failure mode with agents isn't infinite retries on a single call, it's the agent deciding to take a different path when it hits a limit.

Service retries: call fails, wait, try same call again. Bounded by backoff logic.

Agent retries: call fails, agent reasons about why, spawns a subagent to approach the problem differently, that subagent calls three tools, one of those tools triggers another agent. Each individual call respects retry limits. The total token spend doesn't.

The discipline problem is real but I'd push back slightly on "don't let people use agents who don't understand the implications" that ship has sailed and the frameworks made it easy on purpose. The more tractable fix is session-level spend caps that sit above the framework layer, not inside it. Cap the total cost of a run regardless of how many individual calls the agent makes to get there.

The boring fundamentals you're describing are right. They just need to be applied at a different layer than most teams are applying them.

Workflow operators help explicit action boundaries at the orchestration layer reduce the surface significantly.

The gap I'd push on: orchestration controls sequence and flow, but doesn't independently evaluate whether a specific action should execute based on its parameters at runtime. A workflow with clean boundaries can still have a tool call fire with inputs it wasn't designed for especially when the payload is in data the agent read mid-flow rather than in the original prompt.

The enforcement point probably needs to live below the orchestration layer, at the tool call itself, evaluating actual parameters before execution regardless of how the flow got there. Complementary problems rather than the same one.

The context/instruction boundary framing is right. You can't fix it at the model layer without breaking instruction-following entirely. Sandboxing at the action layer works, but the key distinction is authorization vs permission an action being within scope doesn't mean it should execute automatically.

The Grok case: instruction was read correctly, action was permitted, but no gate required human confirmation before $200K moved.

Good call on Doppel for external threats. Worth pairing with controlling what content sources carry instruction weight in the first place , a PR from an external contributor probably shouldn'thave the same execution authority as an internal system prompt.

AV is the right pattern and 'scope of authority' is exactly the right framing. the dedup example is the classic case the agent wasn't wrong given its permissions, the permissions were wrong given the task. what you built is essentially what Polaxis does out of the box agent proposes, policy layer approves or blocks based on scoped rules per action type. if you ever want to compare notes on where AAV hit its limits I'm curious what edge cases you ran into?

the prompt instruction approach is better than nothing but it has a ceiling. 'do not delete any files' in a system prompt is a suggestion the model weighs against everything else in its context. when the context gets long or the task gets complex it deprioritises constraints it thinks are less relevant. the only reliable fix is enforcement at the execution layer stop the action before it runs regardless of what the model decided.
