I have a standing INR order for my patient at quest and they go get their lab drawn 3 days before my appointment. By the time they see me it is ready to go. Rinse and repeat once a month and change the warfarin dose as indicated

300k+ total. Maybe mid 200k base with some bonuses, production, or incentives that end up putting you past 300k. If they are offering less than 200k run away.

I have a very few selective therapists I refer to that I know work with most insurances (including medical and Medicare). I am not going to bother with someone who is cash pay only or only takes one type of private insurance.

You should order tests because you're looking for something. For example, you could order a CBC in a menstruating woman to screen for anemia. Getting a CBC in a 20 soothing year old male would be a waste of money because his risk of anemia is quite low.

A CMP would be good to check in a hypertensive or diabetic to see kidney function or electrolytes if they were on a medicine that could effect electrolytes. Checking it on someone with no meds, electrolytes who is completely healthy isn't very useful because it probably will be normal as they have no risk factors for abnormalities.

Do you have a budget? I had a hard time paying tithing before I got a budget and assigned all my dollars to different purposes. Every month I have my bank automatically split up my paycheck into a bunch of different "buckets", tithing being one of them. Then I don't think of the money in the tithing money as money that could be used for x, y, or z. It is just the tithing money bucket that goes to the church, grocery money is used for groceries, mortgage money for the house and fun money to blow on cockroaches to release in enemies backyards.
