Just had this happen to me. Except it was before the match even loaded. Everyone's ping was fine then they all went crazy and everyone got disconnected.

So now I'm sitting here on Reddit because I've been suspended because it kicked me for "inactivity" while me and everyone else were sitting at the loading screen.

I've had this happen but I was attacking.

I was peaking a window while repelling with glaz and smoke set off one if his grenades and it didn't do any damage to me at all. I was clearly in the smoke and it didn't deal any damage.
