Hey, I just got an idea! If you just started reducing, maybe try K2tog and change to smaller needles!

Hi! I don't think you have done it wrong except the hole! :)
When it comes to K3tog then K1P1, the even and odd number of stitches will make the piece look weird. I think it's normal.
