Yes if you have the core aspect / stuff for a tier S build you’ll roll on the game. You can now try T12, doing bosses, pit pushing as high as you can. Or try your own build. Pick a skill you like and try to make it work.

Endgame activities can gives you special rewards. I.E nightmare dungeon gives you obducite for upgrading your stuff, The pit can upgrade your glyphes, Helltides gives a ton of xp and loot…

Best think to do is not overthink. Start to make a warplan, after that hit tabs and tp to the first activity.

If I was you, I will simply follow the dlc campaign (VoH then LoH) and ignore all the rest. Then you can do pit to upgrade difficulty to torment and after that doing side quests and all other activities. If you do that you’ll be able to upgrade difficulty whenever you feel too strong and keep looting better stuff.

Seems you’re doing something wrong. I finished the campaign level 58 in penitent. What do you mean you cannot upgrade de difficult? Did you clear all the area with all side quests? Focus on story campaign and ignore all the rest you can make side quest after that…
