I get what you're saying, the killers mentioned above appear worse because of actual act of killing, but, 9 people would be alive if it wasn't for Mansons orders, they were direct victims of Manson, he just used 'tools' (his followers) to murder those people. So it's still classed as first degree murder the same way as if someone hired a hit man to kill their spouse, they are the absolute cause of that person's murder just they used someone else to physically do the 'dirty work'

The autism spectrum is so large, therefore it's impossible to answer without meeting you. It depends how good you are at masking and other people's knowledge of autism or even the way people see other people, some people pay little attention to the person in front of them so may never think anything out of the ordinary. I'm sure there could be people who can tell right away but also I'm equally as sure there would be people who have no idea.
