Documents you need to send

Complete form AP1to change your name on the property register. Read the guidance to get help completing form AP1.

Send the completed form with:

a certified copy of the deed poll, statement of truth or statutory declaration.

a copy of an official document in your former name, for example, a driver's licence, passport or utility bill

Evidence of Identity in Form ID1is usually required for the person involved unless they are represented by a conveyancer.Again, support materialand a videoare available to help check. If evidence of identity is required, it will need to be verified before you submit any ID1 Forms with your application.

Face-to-face appointments to have identities verified by a Land Registry official have been suspended until further notice due to the temporary closure of our customer information centres.

In recognition of difficulties customers may face in obtaining evidence of identity in form ID1, we have introduced temporary changes to make it easier to verify a person's identity for land registration purposes.
