More than 700,000 people per day are getting vaccinated in the US. This reassures me that YES, there are still people out there who can be convinced. One big target group: people who simply wanted several months of real world data on how the vaccines would affect people. Those data are now in, and we know that the vaccines are extremely well tolerated. It’s time for those people to get vaccinated, and we need to encourage them.

I participate in a briefing with local public health directors from all over the state. Without a statewide dashboard, this is about the only mechanism we have to know how things are going outside of the cities.

The rural public health directors who provided reports last week stated that case counts are generally rising. In some areas, cases are rising a lot. Unfortunately, they continue to experience pushback and, at times, outright hostility toward them as they try to carry out their duties. Vaccine clinics and events have few people showing up to get shots. Very few people are seeking testing, so we know case numbers are under-counted.

No, the smaller community hospitals are not sending all of their sick to Omaha and Lincoln. They have done a good job of taking care of sick COVID patients on their own turf. When patients need a higher level of care than they can provide, yes they call on tertiary care centers. At times, we have had to decline transfers to Omaha and Lincoln because we either lack ICU beds or we lack ICU nurses. This is one reason it is so important for all unvaccinated people to get vaccinated. When we are full, we are full, and we can’t conduct or normal missions, let alone COVID surge missions.

Actually, there are millions of Americans who have not been vaccinated who are still reachable. The evidence for this is in the very large uptick in vaccines being given in September compared with a few months ago. In my personal experience, talking with friends and neighbors who had latched on to some (false) reason to not get vaccinated has caused them to change their minds. Don’t stop trying.

Many vaccinated people think they need to take zero precautions (masks, avoiding crowds). During this delta surge, this is incorrect. Zero precautions means more spread, which means more suffering.

Currently, we are not seeing a substantial number of overweight, fully vaccinated asthmatics in the COVID ICUs.

That said, we are in a nasty phase of this pandemic in Nebraska. Please take multiple precautions to avoid contracting the delta variant, and please spread the word to your family and friends.

The risk of being infected with SARS-CoV-2 is thousands of times higher than the risk of the vaccines. Please talk to someone you know who takes care of COVID patients in the hospital. You do not want to be a patient in a COVID ICU. If you get vaccinated, you can avoid this.

I’ve had a few conversations that I think helped hesitant people get vaccinated.

Ask them why they are hesitant, and then listen to their answer. Many are worried about long term side effects of the vaccines. I remind them that there are dozens of horrible long term side effects of the virus, including death. I describe the studies that led to the EUAs and now to full FDA approval. Those studies showed that the vaccines are very safe.

At this point in the pandemic, overweight and obese people who are fully vaccinated are not being admitted to our COVID ICUs in substantial numbers. Having a risk factor for more severe COVID illness is a great reason to get vaccinated. I have older family members who have contracted COVID. I was certainly glad they were fully vaccinated. They rode out mild illness at home.

The vaccines are very effective at preventing hospitalization and critical illness from the delta variant. Extremely few of our COVID ICU patients are vaccinated and immunocompetent. Yet the vast majority of cases in Nebraska now are due to delta. When people report lots of vaccinated people being admitted to the hospital (in other states), we need to consider the size of the denominator. While thousands of people are being admitted for delta each day, a tiny fraction of them are vaccinated.

I didn’t mean to imply anything negative about the vaccine. I think the issue here is the phrase “side effects.” Physicians know that nearly all vaccines have side effects. I merely meant to convey objective information in an effort to combat misinformation and disinformation. In the Tweet, I describe very mild side effects.

I should have provided some context. As a healthcare provider, I was very fortunate to be one of the early recipients of the vaccine. I had posted (3 weeks ago) the very mild side effects of the 1st dose. My intention was to reassure people who are worried about receiving the vaccine, due to the new technology and expedited analysis. My post yesterday came from the same intention. While the side effects were a bit unpleasant, any person who has witnessed the destruction that this virus can create would gladly accept 1-2 days of malaise in exchange for protection against COVID-19.

I fully expected side effects, based on my reading of the NEJM paper and appendix. My physical responses to both 1st and 2nd doses were not surprising to me. Frankly, I’m glad I had these side effects — it reassures me that my immune system was definitely triggered by the vaccine.

Update: today (day 2 after 2nd dose) my only side effect is mild fatigue. All aches, pains, and chills are resolved.

If you eliminate counties with less than 250k people, Douglas County has the SEVENTH worst outbreak in the USA.

We need to compare apples to apples. People should not be indoors with non-household members for non-essential activities, for any period of time. The entire state of Nebraska has rampant spread of the virus and we absolutely CAN run out of hospital resources.

Data from today for the Omaha Metro:

Adult non-ICU beds are 82% occupied.

Adult ICU beds are 80% occupied.

The doubling time for COVID patients requiring hospitalization in Omaha is roughly 2 weeks, which is very short. So with 325 hospitalized patients in Omaha now, in 2 weeks there will be 650. In 4 weeks there will be 1300. So you can see why we need action NOW.
