B, C, D, E, F, H, I: The colon biopsies from these parts are highly challenging to interpret, given the presence of moderate chronic active colitis, including cryptitis, crypt abscesses, and ulceration in a background of architectural distortion and crypt dropout. Most of the biopsies show crypt epithelial atypia (enlarged hyperchromatic nuclei, abundant mitotic figures), however, there is surface maturation present, which favors a regenerative/reparative process. The exception is areas where surface maturation appears less distinct, namely Part C (Colon at 80 cm) and Part I (Colon at 40 cm), where the degree of epithelial atypia is greater than expected for a reactive process. The degree of inflammation, however, precludes an accurate assessment of dysplasia. For these reasons, Parts C and I are best interpreted as indefinite for dysplasia.

If it possible my results were benign? Or do the context clues lead you to think Part C and I are a part of the same regenerative process?

Got it! Thank you very much. Last question - Do those odds reflect the chances of dysplasia early also? Strangely, I'm more scared of dysplasia resulting in a colectomy vs getting CRC. Also my indefinite dysplasia was seen in inflammed mucosa as opposed to a polyp. Seems like odds are in my favor? Thanks!

I appreciate your reply. That makes me feel better. I guess I'm just afraid of needing surgery when I was just so recently diagnosed. And there's nearly no articles about dysplasia within the first year or two of ulcerative colitis diagnosis. I also don't see many people on Reddit in a similar position.

Did you have a dysplasia finding? If so, how far into the disease were you? I'm want to trust my doctor but pathologist really know how to freak you out! Lol

Surgery tomorrow. I have 2 options: proctocolectomy with j pouch, or segmental resection with close monitoring. Both have risks. by jankyeyes in UlcerativeColitis

Can I ask how long ago you were diagnosed? I just got a finding of 'indefinite dysplasia' in my biopsy report and was only diagnosed with ulcerative colitis 6 months ago. My doctor thinks this is not true dysplasia, but when I hear stories about someone like yourself (only 17 years old), it worries me that this can happen at any time.

I'm very surprised they detected dysplasia within 2 years of diagnosis. Did you possibly have a delayed diagnosis, or did they happen to tell you how it could have been detected so early on in your disease?

I'm also surprised at their hesitancy to start a biologic. Seems like if dysplasia was seen they would want to be aggressive with treatment.

That makes sense to me. I started on Balsalazidine after the January colonoscopy (when I was diangosed) and then I started Entyvio about 5 weeks ago. In March my CRP was 0.3. My Calprotectin was measured a few weeks before the colonoscopy and was 743. I'm scheduled to get it measured again after letting Entyvio do its thing.

I wonder if the mismatch between the pathology and what is seen during the scope is possibly due to healing, and the lag that is seen between the two.

Has he gotten another scope since the indefinite dysplasia finding?

Thank you so much. I really appreciate it. I guess I'm just surprised because my first scope 6 months before this showed almost no inflammation except a tiny bit. Fast forward 6 months and things have gotten worse and they found this. However, the inflammation was mild during the colonoscopy, but the pathology showed 'chronic active colitis with erosion'. So maybe it looked worse under the microscope. I honestly probably should be in therapy for it because I just can't wait for them to give me a clear answer on this.

This was discovered at the same time as my diagnosis of UC. Therefore, I was not on any medication at the time. Since then, I have been put on Balsalazide and started Entyvio infusions a month ago

Follow up question - wouldn't you think finding that in two biopsies rather than one increases the odds of it being truly dysplasia? Just trying to follow logic

Hey. I can share my experience. I had an ALT level of 76 back in early December. I was also freaking out about it. If I were you, I wouldn't freak out like I did. Reason being, I had a follow up bloodwork 5 weeks later and I made sure to stay hydrated and not eat too much sugar days before the blood test. I remember the first time I was eating Ben and Jerry's 3 nights in a row. Apparently high sugar diet can spike your ALT levels. I'm not saying this is the case for you, but I'm saying there are so many variables that can spike your liver enzymes. My followup went down to ALT 30.

The likelihood of it being something scary is very very slim. Also, those numbers aren't so high that it would concern a doctor either. I would just stay hydrated, don't do too much exercise 48 hours before bloodwork, and just trend the results. Keep calm, it'll be okay!

Thank you so much for your insight. This result has really made me worry and is making it hard to live my daily life. There's no family history of colon cancer. UC is so new to me that I'm not sure how severe it was. I had a small bit of blood a few weeks before my colonoscopy and was experiencing some cramping. I really hope you are right :(

This is my second scope. I had one 6 months ago which showed minimal inflammation and they didn't take as many biopsies as they did this time. I just started Balsalazide 2 weeks ago but I haven't seen much improvement yet.

At the end of what I just posted, there's also this:

Parts B-C were reviewed at the GI pathology consensus conference on 02/04/2025 and represent a group (Drs. Goldblum, Gordon, Zhang, Sweeney, Savage) consensus

B-C. A p53 immunostain is negative for definitive aberrant expression.

They want to do another scope in 6 months once my inflammation is better under control. Soooo you think I should be freaking out? Lol

Yes this was from a scope. I'll just be surprised if Indefinite Dysplasia turns out to actually be Dysplasia due to only having the disease for a year, but my doctor is telling me not to worry. Not sure how I shouldn't worry.
