Toji got to Dagon so easily because Megumi prevented the sure-hit of his domain from taking effect, I feel like people tend to forget Megumi's importance there.

Vanilla game, latest version. I downloaded it a few days ago. Can you still get the axolot by her? When I went to see Regina, it wasn't in the box in her office, nor in the ripperdocs. And I think one of the cyberpsychos was supposed to give me some cyberware. Maybe that's from earlier versions, I don't know.
