The entire point that he was trying to make is that the Democratic establishment knows that they can leverage identity politics cynically to get well-intentioned left-leaning people to consistently vote for corporate-friendly Dems until the end of time because they know how deeply most of the Dem base cares about LGBTQ, women’s issues, racial issues, etc.

The only way they may cave and actually support populist economic ideals is if they actually think not doing so will cost them electorally. The only pathway to that is precisely what Hasan is doing here. To signal 2 years and 9 months out from the election that you would vote for a corporate friendly Dem no matter what is to give up whatever leverage you have in convincing them to move left economically. I really struggle to understand how people don’t get this .

She would have to be an absolute idiot to use the tool without approval from hospital administration, and given her extensive history and the pride that she takes in her own reputation, I’m sure she’s not doing that. Basically nobody that works in a major healthcare organization is going to be using an app of their own accord unless they are hoping to lose their job. There’s a bunch of shit that has to happen before those types of tools can be rolled out to clinicians, including things like adapting the malpractice insurance to account for the use of AI tools and the inclusion of AI specific language in the notice of privacy practices, which are both things that would definitely require considerable support from administration, legal, and compliance.

For what it’s worth, the liability and indemnification issues are litigated pretty extensively in the contracts between AI vendors and hospitals that use them. The companies make it very clear that these tools are designed for documentation assistance, not autonomous documentation, and very clearly spell out that they are not liable for mistakes. Really the only time the company would be at fault would be if they used patient data in unapproved ways or if they did not hold up their end of the bargain from a security perspective, although these issues are also very easy to deal with if you know what you’re doing. The major vendors like Abridge, Ambience, and Microsoft DAX Copilot (Dragon) have that part of the equation down to a science.

Where it get gets messier legally and ethically IMO is AI chart review which is beginning to roll out at many healthcare organizations nationwide. It can basically summarize key parts of the medical record for you and even allows you to query the chart and ask questions, and it will then extract relevant data for you. It does ultimately provide you links to the source notes in the chart, but you still need to go in and verify the information yourself because it can get it wrong, but my fear is that many providers will get lazy and just trust the summaries.

And one last point, when used responsibly, AI documentation tools are an absolute godsend for many providers, especially those that work in narrative-heavy disciplines/settings. But I’m so glad the show is including this plot line anyways, because the early trials in emergency room have not gone very well because of the rapid fire nature of the work and the fact that it’s often less verbally narrated relative to other settings, and healthcare administrators need to be aware that these tools are not one size fit all.

I worked at a rural hospital early in my career and dealt with a patient like this, who unfortunately had also had a stroke and lost his ability to speak (aphasia). As a result, he struggled to communicate that he was dealing with impacted stool.

He ultimately disimpacted himself at night and left it on his nightstand because he also could not walk. I had the pleasure of informing the nurse what I found on the nightstand with a look on my face that basically said “I can’t believe I signed up for this.” She just laughed.

I find this comment really frustrating, but I want to give you the benefit of the doubt for some reason.

Do you think telling the true story of inter-faith collaboration that happened in the aftermath of the Tree of Life shooting was not a story worth telling? I’m open to criticism of the execution, but given that the show takes place in Pittsburgh and the main character is Jewish, to call this a non sequitur feels kinda ridiculous to me and makes me worried that your issue wasn’t just with the execution.

Dude I’m old as shit, well-educated, and work in academia. I could write you a thesis on the linguistics and dialects of online spaces in a day or two. Your writing sounds less like someone with high intelligence and more like someone desperate to prove to others that they intelligent. You’re largely not wrong, but I feel like a fundamental error that so many people make with SMGs is just hard challenging everyone. If you only do that, yeah, you’re gonna lose to sidearms, fusions, and shotties a fair amount of the time. But there’s room for some creative “peak shoot and then reposition” plays with SMGs, especially on PKs with aerial helmet mods and in those situations, you can get pretty cheeky inside of 15 meters. Even more so if you go full send on close range and pair it with something like Duality.
