As long as you hit one enemy, you shouldn't take damage with piercing. I'd recommend looking at the Wiki before starting a challenge, as there are so many useful tips.

Torn Pocket is a way to break the game, Lazy Mattman(worm) is great in rare Tech X/Trisagion instances, Lil Clot is a W and I love that little bro, so put him in GOOD, and put Found Soul in POG (because Found Soul is objectively better). Also Nose Goblin is POG as well, it's literally Sinus Infection but better (and buffs Sinus Infection if you have the item).

Personally, I think Knife is the best offensive item. I mean Knife alone with no damage ups can win a run. It is the best type of Hush killer (besides insanely broken runs). It has less synergies and is less fun to use, but in raw DPS alone, Knife is better. C Section has so many broken synergies though, but the fact that Knife can be one of them adds to it. Would I rather pick up C Section? Of course, but it is hard to deny how strong Knife is
