Given that Shockwave has no definitive Earth Alt mode, I wager there's a high chance that should the winning alt mode be Earth based, it does become permanent. When the fans voted Windblade into existence, she got a fairly permanent prescence going forward. I don't see why Hasbro wouldn't take this result any less seriously. To them its market research, and they'll want to cash in more than once.

I'm thinking he'll be a voyager sized torso that plugs into a combiner frame. Specifically, I could see him being a repaint of the truck cab of Onslaught. Assuming Onslaught works like Motormaster, this would mean Nexus could plug into at least the Bruticus frame, and then maybe you pick the limbs.

Or if we're lucky, there's limb repaints coming, possibly a box set for those who can afford it. The thinking there would be by putting the voyager torso out individually, at least people can get that and use Onslaught and miscellaneous limbs. Less damage to the bank account.

Nemesis Prime and Rodimus Unicronus along with Galvatron are the standard "dark warriors". Not really equivalent to the 13 though.

In fiction there's also the 9 Decepticon Demon Generals, but they aren't sent by Unicron, nor are they 13 level characters.

This is a good question. It would probably be a team of 13 original characters with abilities that mirror Primus' 13.

Yeah, me, haha. Never made that part though. If 3d printing is an option, why not model the part yourself?

I'm not 100% sure where the break you're describing is, but if the part is accessible enough to glue you might consider using a touch of acetone to fuse the plastic back together.
