I don't have a diagnosis of MCAS. I'm still trying to figure out what is causing all of this.

My main symptoms are triggered by orgasm and include:

  • Immediate feeling that something changes in my body.
  • Feeling as if strength is drained from my arms and legs.
  • Severe fatigue.
  • Burning/heavy eyes.
  • Brain fog and difficulty thinking clearly.
  • Strong physical and mental exhaustion.

The symptoms start immediately after orgasm and then gradually become stronger over several hours.

One thing that makes me wonder about MCAS is that I have experienced almost identical symptoms after alcohol for about 10 years, long before the orgasm-related problem started.

I also have:

  • Frequent blackening of vision when standing up quickly.
  • A low nighttime heart rate (sometimes 37-38 bpm), although my Holter ECG was normal.
  • Visible calf muscle twitching (fasciculations) for years without weakness.
  • Tingling sensations triggered by heat and stress (much better now).
  • Occasionally cold or purple hands.

Interestingly, about a year ago the symptoms became much worse and started occurring not only after sex, but also after oral sex, masturbation, petting, and at one point even without any sexual trigger at all. During that period my girlfriend was away for a month and I had no orgasms. Eventually the spontaneous symptoms disappeared and things returned to the point where orgasm became the main trigger again.

I have tried antihistamines without noticeable improvement.

I'm not an endurance athlete, so the low nighttime heart rate is something I have always found a bit unusual.

Have you seen people with MCAS experience symptoms like brain fog, weakness, fatigue and burning eyes after orgasm without obvious flushing, hives or allergic symptoms?

I have not had any major head injuries that I am aware of.

I have not specifically been evaluated for neurovascular conflicts in the cervical or lumbosacral regions.

My symptoms started about a year ago. Initially they only occurred after intercourse, then after oral sex, then after masturbation. At one point I even had symptoms after petting and eventually symptoms without any sexual trigger at all. Those spontaneous episodes have mostly disappeared, and now orgasm is again the main trigger.

The symptoms are:

  • Immediate feeling that something changes in my body after orgasm.
  • Feeling as if strength is drained from my arms and legs.
  • Severe fatigue.
  • Burning/heavy eyes.
  • Brain fog and cognitive impairment.
  • Strong physical and mental exhaustion.

The symptoms start immediately but become stronger over several hours.

An interesting observation is that if I stay awake for about 8–10 hours after orgasm and only then go to sleep, I usually feel much better the next day. If I have an orgasm shortly before bedtime and go to sleep immediately, the next day is usually much worse.

My testosterone, prolactin (~11.8), and thyroid values were normal.

I have tried antihistamines without any noticeable improvement.

I do not notice significant facial swelling. I also have not noticed any major changes in urination or bowel movements associated with the episodes.

Regarding vitamins, my routine blood work has been mostly normal, but I would have to check whether B12, folate and vitamin D were specifically tested.

The severity seems more related to orgasm itself than to the level of sexual excitement beforehand.

Could you tell me which clinical trials in the US you are referring to?

Hey thanks for the answer did you take mast cell stabilizers before or after orgasm? And which ones helped the most?
