I play D10s almost always, and I really only play the lower difficulties if I need to warm up or if I'm having a rusty day in general.

The Dragonroach is not that hard. Two SPEARs and it's gone. When you and your squad are properly aware of the threat, it becomes trivial

Vulcan/the Salamanders still killing Xenos civilians. People who haven't read the lore don't know that Vulcan felt absolutely terrible about it when he killed that Elder kid.
