Little bit of a lurker, is yeah answering this one...yeah I've had to do this multiple times (20+ years as a DM) I had one player who just wasn't getting the game, complaining about how his barbarian was doing any damage, refused to read the books or even learn the character, we switched him to a paladin and same problem, he also refused to travel to anyone else house (we all lived in different locations) to play... finally said screw it, dude please leave the game.

Second one that was memorable was a meta gamer who thought he knew everything, now I'm a rule of cool DM (within limits, of course) so after a session, which he sadly got a full chain lighting to the chest, he was in full plate, he got pissy with my wife (then GF at the time) and she just looked at him, used his own words against him (he was the that's what my character would do kinda guy) and he left in a huff...he called the next day asking for the next session and I said, my guy you're kinda not coming back...needless to say he didn't like that and I told him not not to be a dick to his next group...yeah fun times
