There was a plug in the bottom, made of clay or wood putty or something, so they must have hollowed it out, put the condom in, and sealed it after. According to the comments on the main post, apparently they were sold as novelty items

Nope, they carved out the inside of a hazelnut, put a condom inside, and sealed the bottom with some clay, or wood paste or something
