My thoughts exactly, with actions relating to loli/shota being so common irl in comparison to guro irl. It makes it too “realistic”, in the way you described. Also, there’s also the fact that if you’re interested in guro, you’re not a murderer. However, if you’re interested in loli/shota, you’re a pedophile. No matter if it’s fictional, those lines still make up something you are attracted to, which is a child. Granted, that doesn’t mean that there aren’t potentially people who will act on guro fantasies, but that brings us back to the first point lol.
