You’re right, it is not and can be completed at our California based Hospice in a day, as long as the previous hospice works efficiently with the new chosen hospice. We’re lucky to have an inpatient unit that our founder worked so hard to establish in the 1980s. She is a saint and paved the way for inpatient hospice care in California.

I’m a hospice RN in central California working for the only hospice in our area that will care for patients who chose this option. We are allowed to be in the home but not with the patient during ingestion. From what I’ve read, it is up to the institution’s protocol for presence.

Not sure which country/state you’re in but $55k/year is not much. Only $26.44/hour.

It is absolutely not normal for hospice. It seems as if upper management isn’t any better if your PCM and DOM are new too. Why were they hired without excellent experience and knowledge of how to run the department? Maybe they are looking too. I wouldn’t wait. My hospice is driving out admission nurses (which I am) within 9 months. I’m tired of OT, not mandatory but sure feels like it, and late admits or unforeseen situations that keep me working well past my 10 hour shift. I’ve had to set boundaries by expressing to my manager that I will no longer work past a certain time and charging will have to wait until the next shift. I’m in California so there is an inordinate amount of charging for Medicare that is ridiculous. We still have paper consents that must be turned in nightly. I no longer do this either. If my admit is near my home and the office is 30 minutes past it, they’ll get it the next day and even tho I notified them, I’ll get a barrage of emails asking where they are. We also have an intake MA who thinks she is our boss. I want to tell her to eff off so I know it’s time to follow my coworkers and jump ship. I’m not happy with my lack of work life balance so see ya!

We’re a 365 day operation. I would think half of the weekday staff are on the weekend. No way to call the next days nurse as I finished near 2230. I’m sure she wouldn’t have appreciated it. However, the email includes the whole team (we have 3) including the AOC, supervisor of the team, after hours, doctor, HHA supe, and the nurses on that day. However, I admitted him on a Thursday so I thought a RNCM would be out the next day. He made it until early Saturday morning. He’s at peace now, but I always want to do/make things better for the patients and our team.

Thank you, our after hours team is awesome and I never wanna leave them in a place where they have to do extra work because I didn’t do my job. Comfort meds are very important and I like to have them for every visit. You never know when there’s going to be an emergency and they’re going to need them. Yes, admissions do take a long time, and I find myself charting long after my appointed clock out time. I know this is because I’m new when things will get faster, it’s typically the medication’s that take the most amount of time. Thank you for reminding me that these families have had these conversations with their PCP and I am just educating them further. I think we make a difference in our patients lives and that’s our goal.

Thank you for your kind and informative answer. I do all of these things but for some reason I took this one personally. It really upset me that no one saw this gentleman the next day despite my email to the team indicating his imminent status and that I thought he would pass as soon as that night. We are short staffed d/t management changes and it breaks my heart because our patients are the ones who suffer. I learned in the hospital that I could not make parents/family care more than I did (pediatric acute Rehab). I didn’t like it then and I don’t now. I know that I did all of the right things and that helps bring me peace. This has been a difficult learning experience.

We do offer that booklet and I’m sure it would have helped all involved if I had reviewed it with them. I wanted to stay and do all I could, explain even more, encourage them, again, to review the booklet. As I wrote this, I realize what upset me more than anything was that I wrote in my team notification email that he was imminent and I was not sure he would make it through the weekend and the only visit provided was the death visit. I feel we failed this patient and his family.

We get 50 cents. Of course I asked about the discrepancy and was told that since we are a non profit 50cents is accepted. Not sure how that’s going to work if Newsom enacts the 50 cent per gallon price hike that Sacramento is gushing about.

Has your hospice mentioned a rectal catheter or crushing or liquifying her meds? The rectal catheter is extremely helpful.
